Please complete this form and mail or give it to the Church Administrator, or leave it in the green basket on his desk. Your reservation will be confirmed by the return of a copy of this form to you. Confirmation places your reservation on the church calendar.

Please note: You are responsible for your own set-up and clean-up. (Trash disposal is outside the Ewa back door.) Since the church building is used by many different groups, parking space will be assigned by the church office if there are multiple users. If you cancel or change your event, please notify the office as soon as possible so we will know the space is available for someone else. Hours of use of the building are 7:00 a.m. to 10:00 p.m. Please turn out lights in the area you use and close outside doors except for the front door.
